If I have a different address, phone number, or emergency contact what paperwork needs to be completed?
Please complete the Employee Address/Emergency Contact Change Form then forward it to your Payroll Clerk.
May I change my W-4 form? When and how?
Ask your Payroll Clerk for a new W-4 form. Make the changes and return the form to the clerk. A pay action form will be completed, the W-4 form attached to it, and then both sent to the Human Resources Department. Do not send only the W-4 form, it will be returned to you to be processed by your Payroll Clerk.
If I get married or divorced, what paperwork needs to be completed?
Your Payroll Clerk will have you complete a new W-4 form. They will complete a Pay Action Form and submit both documents to the Records Administration Section of the Human Resources Department.
If your last name changed, additional documentation, such as the official court order, your updated Social Security Card and your updated Driver's License will be required. All of these documents will need to be submitted to your department Payroll Clerk. You will also need to get a new ID Card by contacting our Records Administration Section at 464-3082.
