The Supplemental Questionnaire
(also known as the Unassembled Exams)

This is a brief overview on the Supplemental Questionnaire , also known as the Unassembled Examination, process works:

• For this type of examination the Application form and Supplemental Questionnaire is the exam.
  
  
• Detailed instructions on how to complete the online form is included on each form. Read and follow these instructions carefully. It is very important that you thoroughly fill out the entire online Application form and Supplemental Questionnaire.
  
  
• Responses such as "see resume" will not be considered.
  
  
• The information you provide regarding your education, training and experience will be evaluated and compared to an established scale.
  
  
• The online Application and Supplemental Questionnaire must be completed by the “Apply By” date on the “Current Job Openings” web page. Failure to do so will result in your disqualification from placement on the eligible register.
  
  
• In order to receive a passing grade you must meet the minimum qualification requirements for the job classification. The minimum qualification requirements are indicated in the Job description and the recruitment announcements which can be found on the “Current Job Openings” web page.
  
  
• Important:

  If your supplemental questionnaire states that you are required to take a written examination, you must contact the Human Resources Department prior to the "Apply By" date to schedule your exam date and time. We have provided information regarding the written test on the Written Examination web page.
