The County’s life insurance plans are administered by The Hartford. To review all of the provisions of the group term life plan, please view the Term Life Certificate .
Basic Coverage:
The County provides basic Group Term Life Insurance to all permanent employees working at least 20 hours per week at no cost in an amount equal to your annual salary rounded up to the next $1,000.
Supplemental Coverage:
Supplemental life in addition to the basic coverage may be purchased.
- If purchased at time of initial enrollment, no medical underwriting is required, provided the optional coverage does not exceed three times the base salary.
- Supplemental coverage is available in multiples of $5,000, not to exceed five times annual salary or $250,000, whichever is less.
- Rates for life insurance coverage will be based on age groups.
- When you move to a new age group, the new premium will be deducted the first pay of the month of your birthday.

Excess coverage or a late application is subject to proof of insurability. Complete the Proof of Insurability Form, print and sign the completed form, and send it to the Employee Benefits Office (Annex Building, 400 S Ft Harrison Ave, Clearwater FL 33756) for processing.

Note: There is a "Reduction in Coverage" provision built into both the Basic and Supplemental life insurance beginning at age 65. Please review the reduction schedule within the Term Life Certificate .
Life Insurance Beneficiary Change:
If you want to change your life insurance beneficiary, you may do so at anytime. Please keep this in mind should you have a change of status event during the plan year. If you would like to change your beneficiary, complete the Beneficiary Change Form, print and sign the completed form, and send it to the Employee Benefits Office (Annex Building, 400 S Ft Harrison Ave, Clearwater FL 33756) for processing.

Accidental Death & Dismemberment (AD&D) Provision
An AD&D (Accidental Death & Dismemberment) provision applies to both the employee’s County-paid basic coverage and employee-paid supplemental coverage. This benefit increases the amount payable for certain types of Dismemberment or accidental death.

Optional Coverage for Spouse and/or Child(ren):
AD&D does not apply to spouse/children optional coverage. Optional Dependent Life Insurance does not require a beneficiary designation. The employee is the beneficiary. Also, no spouse or dependent names need to be listed for enrollment - be sure to refer to the definition of dependent in the Term Life Certificate to make sure you aren't paying for coverage that doesn't apply.

Group Term Life Waiver of Premium Provision
The group term life insurance plan contains a provision to waive premium and continue coverage for employees who are totally disabled for at least 6 consecutive months. There is also an accelerated death benefit to provide immediate payment for terminally ill employees with a life expectancy of 12 months or less.

Upon leaving County employment there are a few options to be able to continue your life insurance coverage.

Wrap Plan Group Universal Life Coverage
This plan is no longer accepting new enrollments. Employees that already have this coverage should call WrapPlan Customer Service at 1-800-322-0426 for information or assistance with their coverage.
