The Financial Assistance Program helps residents with documented medical disabilities. Residents work with compassionate case managers to achieve economic self-sufficiency.

You must meet these three conditions to qualify:
- Be disabled and pursuing Social Security Disability and/or Supplemental Security Income (SSI) benefits. You must not be receiving federal disability benefits.
- Have income less than $450 a month for one person
- Own assets less than $500 for one person
If you are approved for this program, you must attend a consultative exam by an independent medical professional, to assess your medical and employability status.
- Up to $450 a month for help with rent, mortgage payments, utilities and food
- Assistance in applying for/obtaining Social Security Disability and Supplemental Security Income (SSI) benefits.

How do I apply?

Complete an application. To obtain an application, you may:
- Print an application online.

- Call one of our offices to have an application mailed to you.
- Visit one of our offices and pick up an application.
- Print an application online.
- Look at the application for the paperwork you need to bring.
If you have questions, please call our office Monday through Friday between 8 a.m. and 5 p.m. This might save time and help us serve you better during your visit.
- Visit an office to turn in your application. You will be seen on a first come, first-serve basis. If your application is approved, future meetings with a case manager will be made by appointment.

What paperwork do I need?
- 1. Documentation from a doctor that you are unable to work.
- This documentation must include your diagnosis and length of time unable to work.
- Copies of any medical records you have is helpful.
- 2. Two forms of ID for you and your spouse - Examples:
- Birth certificate
- Florida identification card
- Social Security card
- United States Passport
- Valid driver’s license
- Voter registration car
- 3. Two documents that prove you are a Pinellas County resident - Examples:
- Bank statement with current address
- Motor vehicle registration or title with current address
- School record
- Pay check or pay stub
- Rent receipt
- Utility bill - Examples: electric, water, gas, cable
- Voter ID card
- 4. Proof of legal citizenship or immigration if born outside the United States:
- U. S. Naturalization papers
- Resident card
- 5. Proof of income for the past 60 days - Examples:
- Award letters from the Social Security Administration or Veterans Administration
- Child support/alimony
- Pay stubs
- Unemployment insurance benefits
- Worker's Compensation
No income? Bring a support statement from someone who is helping you if you have no income. If you claim you have no income and no one is helping you financially, you will need to show proof of how you are paying your living expenses.
- 6. You will need to provide verification of your living expenses from the past 30 days.
- 7. Proof of assets - Examples:
- Bank or credit union statements
- Life insurance cash value
- Non-homestead property
- Retirement/401K funds
- Stocks, bonds, trusts
- 8. Status of any other assistance applied for - Examples:
- Supplemental Security Income, Social Security Disability(SSI/SSDI)
- Unemployment Compensation
- Worker’s Compensation
- 9. Status of legal action or insurance claims in process.
